They are both dedicated to patient wellness and education, and are available for medical and wellness visits specifically addressing chronic and ongoing conditions.

Pricing Details

Primary Care/Chronic Care Pricing

  • Initial Visit (includes in-person exam, lab orders with follow-up call for lab review): $390

  • Routine Scheduled Check-up (no changes to medications or plan of care): $50 

  • Simple Follow-up Telehealth (1 condition, changes to plan of care): $100

  • Complex Follow-up Telehealth (2-3 conditions addressed or plan of care modified): $140

  • Comprehensive Follow-up Telehealth (Greater than 3 conditions addressed): $160

Chronic care pricing includes access to your provider via text at no charge. If questions cannot be properly addressed via text, patient will be notified. All follow-ups requiring modifications to plan of care will have an associated charge as detailed above unless previously discussed with the provider.
